Professional Staff Roster

Director: Gordon P. Wolfe, M.S.W., L.C.S.W., N.C.G., C.M.C. Founder and co-owner of H.N.S., has 40 years post masters experience. 25+ years experience in guardianships, conservatorships, and working with the elderly and disabled. Active in the National Association of Professional Geriatric Care Managers (NAPGCM) and the National Guardianship Association (NGA) and past member of National Boards of Directors of both organizations. Holds National Certifications in both Guardianship and Geriatric Care Management. Trained in Contemplative End-of-Life Care (CEOLC).
Care Manager: Chris Reifenrath, P.T., M.A. Gerontology, C.M.C. Has 18+ years experience working with seniors in hospital, home health, and skilled nursing settings as a physical therapist. Joined HNS in 2002. Now a co-owner of HNS, Chris specializes in complex cases with multiple medical issues. She is also skilled at helping clients and families cope with grief, loss, and end-of-life issues. Chris is the 2010 President of the Board of Directors of the Western Regional Chapter of NAPGCM. Chris was co-chair for the 2009 WRGCM Conference. She is a Certified Geriatric Care Manager and is also certified in Compassionate End-of-Life Care (CEOLC).
Care Manager: Erin Custer-Dougher, B.A., M.A., Gerontology C.M.C., R.C.F.E. Erin is an expert in long-term care options. Post-masters experience includes nursing home, social services, and assisted living administration. She came to HNS after several years as Health Services Director at Heritage Club of Denver. Erin is a licensed assisted living administrator. She is also fluent in Spanish. Erin is a Certified Geriatric Care Manager, and was on the planning committees for both the 2007 WRGCM Conference and the 2009 WRGCM Conference.
Care Manager: Jennifer Pagano, B.S., M.A. Gerontology. A Denver native, Jennifer joined HNS in 2007 after 7+ years experience as a PGCM with Open Care in Charlotte, N.C. Jennifer’s specialty is dementia care, including training for family and professional caregivers. Jennifer has also worked at the Alzheimer’s Association and helped develop an adult day program. She was voted Outstanding Member of the Southeast Chapter of NAPGCM in 2005 and was co-chair of the WRGCM 2009 Conference Program Committee. Jennifer is also a Certified Geriatric Care.
Care Manager: Gretchen Alston, R.N., B.S.N. Part-time care manager with HNS since October 2005. Gretchen has 35 years of nursing experience, including recent service at Rose Hospital in the Preoperative Care Center where she did assessments of biopsychosocial and spiritual needs of surgery patients, many of them seniors. She served on the Rose Hospital Bioethics Committee 2001-2005 and was the chairperson of that group in 2004-2005.
Care Manager: Su Zerwin, R.N. Extensive experience in home health care, adult day care, and senior housing, with additional nursing experience in oncology and mental health. RN for 40+ years. Care Manager with Human Network Systems since 1992. Semi-retired but available to HNS on a limited basis, provides fill in coverage and covers a small case load..
